Complete Enrollment Paperwork
Once you're ready to enroll, our team will walk you through all required forms, health records, and program agreements.
Welcome Your Child
We'll work with you on a gentle transition plan so your child feels safe and excited from day one.
Enrollment Requirements
To complete enrollment, families will need to provide the following. Our staff will help you gather and organize everything.
- Completed enrollment application
- Copy of child's birth certificate
- Up-to-date immunization records
- Health assessment / physical exam form
- Emergency contact information
- Signed program and policy agreements

CCDF Assistance
We understand that quality child care is an important investment. Bambini is proud to be CCDF registered to help eligible families access financial support.
- Bambini is CCDF (Child Care Development Fund) registered
- Eligible families may use CCDF subsidies to offset care costs
